New Client Information

We welcome new patients!

In order to create your pet's medical file please download and fill out the Pet Medical History PDF and New Client Information PDF.   

 

Please  Email them to Foxridgevet@yahoo.com prior to the appointment.   

Please have your pets medical history Emailed to us at least 3 days prior to the appointment .  Please Email to  foxridgevet@yahoo.com

Also bring a list of all the medications and supplements your pet is currently taking with you to the appointment.

Cancellation Policy

If you need to cancel an appointment please call us at least 24 hours in advance to reschedule or cancel (phone number 630-859-0471).  If you reach us after hours please leave a message on the answering machine.  Early cancellation will allow another patient access to  timely care.

Credit or Debit Card Processing Fee
 

Effective January 1, 2025 a 3% processing fee will applied to any invoices paid using credit or debit card. We appreciate your understanding as we implement this change to better serve our customers. If you have any questions or concerns, please feel free to reach out. Thank you for your support.  To avoid the fee please pay with cash or personal check.

No-Show Policy

​A no-show is a client who misses and appointment without cancelling it.  Missed appointments will require prepayment of the exam fee when rescheduling. When the prepaid appointment us kept, the fee will be applied to the invoice. If the prepaid appointment is not kept, the fee will be forfeited.  If you miss 3 appointments, we will terminate our veterinary-client- patient relationship.
